Privacy Policy

Arizona Healthcare Help is deeply committed to protecting the privacy and confidentiality of every person who interacts with our organization. This policy describes how we collect, use, protect, and disclose personal information gathered through our website, patient intake process, and navigation services. Last updated: January 15, 2026.

1. Information We Collect

Arizona Healthcare Help collects personal information only when necessary to provide patient navigation services, benefits enrollment assistance, prescription assistance program applications, charity care submissions, and medical debt counseling. The categories of information we may collect include:

  • • Contact Information: Name, phone number, email address, and mailing address provided during intake or through our website contact forms.
  • • Financial Information: Household income range, employment status, and insurance coverage details required for benefits eligibility determinations and program applications.
  • • Health Information: Current medications, medical conditions, and healthcare needs disclosed during patient navigation consultations, used exclusively to match you with appropriate healthcare resources.
  • • Documentation: Copies of pay stubs, tax returns, insurance cards, medical bills, and other documents you voluntarily provide to support benefit applications.

2. Information We Never Collect

Arizona Healthcare Help explicitly does not collect, record, store, or transmit the following categories of information under any circumstances:

  • • Immigration Status: We do not ask about, record, or report citizenship status, visa status, country of origin, or any immigration-related information. Our services are available to all community members regardless of documentation status.
  • • Social Security Numbers: While certain government benefit applications (such as AHCCCS) may require a Social Security Number, this information is entered directly into the government portal during your appointment and is never recorded in our internal client files.
  • • Criminal History: We do not request or record any information about criminal history, arrests, or legal proceedings.

3. How We Use Your Information

Personal information collected by Arizona Healthcare Help is used exclusively for the following purposes: determining your eligibility for healthcare programs, benefits, and assistance resources; completing and submitting applications to AHCCCS, KidsCare, manufacturer Patient Assistance Programs, hospital charity care programs, and other healthcare access programs on your behalf and with your explicit written authorization; coordinating referrals to community health centers, specialists, and support services; providing ongoing patient navigation including appointment scheduling, medication refill tracking, and care coordination; and generating anonymized, aggregate statistical reports for grant compliance and community impact measurement. We never use personal information for marketing, advertising, fundraising solicitation, or any purpose unrelated to direct patient navigation services.

4. Information Sharing & Disclosure

Arizona Healthcare Help does not sell, rent, trade, or otherwise disclose personal information to any third party for any reason, with the following limited exceptions:

  • • Authorized Program Applications: When you provide written authorization for us to submit a benefits application on your behalf (AHCCCS, PAP, charity care), we share only the minimum information required by that specific program.
  • • Referral Coordination: When we refer you to a community health center or specialist, we share your name and contact information with the receiving provider only with your verbal or written consent.
  • • Legal Requirements: We may disclose information if required by a valid court order or subpoena. We will notify you of any such request unless legally prohibited from doing so. We will never voluntarily provide information to immigration enforcement agencies.

5. HIPAA Compliance

While Arizona Healthcare Help is a community-based nonprofit and not a HIPAA-covered entity (we are not a healthcare provider, health plan, or healthcare clearinghouse), we voluntarily adopt protections that meet or exceed HIPAA Privacy Rule and Security Rule standards. All staff members complete annual HIPAA training, sign confidentiality agreements, and are subject to disciplinary action including termination for unauthorized disclosure of client information. Client records are maintained in encrypted, access-controlled systems with individual user authentication and audit logging.

6. California Consumer Privacy Act (CCPA) Rights

Although Arizona Healthcare Help is based in Arizona, we extend CCPA-equivalent privacy rights to all clients regardless of state of residence. You have the right to: know what personal information we have collected about you; request a complete copy of your client file; request deletion of your personal information from our records; opt out of any future data collection; and designate an authorized agent to exercise these rights on your behalf. 7. Website Data & Cookies

Our website (arizonahealthcarehelp.org) does not use tracking cookies, analytics pixels, advertising networks, or any third-party data collection tools. We do not track individual visitor behavior, collect IP addresses for identification purposes, or build user profiles. Interactive tools on our website (such as the Benefits Eligibility Quiz and Provider Search) process all data entirely within your web browser using client-side JavaScript. No information entered into these tools is transmitted to our servers or any third party. Contact and intake forms submitted through the website are transmitted via encrypted HTTPS connection to our internal case management system and are subject to the same confidentiality protections described in this policy.

8. Data Retention & Destruction

Client records are retained for a minimum of three years following the most recent service interaction, as required by our grant compliance obligations. After the retention period expires, all client records including paper files, digital records, and application copies are securely destroyed using cross-cut shredding for paper documents and Department of Defense-standard digital wiping for electronic files. You may request early deletion of your records at any time by contacting our office, subject to minimum grant compliance retention requirements.

9. Zero Data Selling Guarantee

Arizona Healthcare Help has never sold, will never sell, and is contractually and ethically prohibited from selling personal information of any kind to any third party under any circumstances. This prohibition applies to all categories of information including contact information, financial data, health information, and website interaction data. This guarantee is unconditional and has no exceptions.
